Volunteer Requirement(Required)

I acknowledge that if my child is on the Creekisde Swim Team, that I must volunteer AT LEAST at one swim meet (snack bar, timer, etc).

Code of Conduct(Required)

I acknowledge that as being part of the Creekside Swim Team, children and parents must adhere to the Code of Conduct (posted on the website). Children and/or Parents not following the Code of Conduct may result in suspension or removal from the swim team.

Fees(Required)

One child returing (any age) ($100)

Two children returning (any age) ($200)

Three or more children returning (any age) ($250)

This is my child’s first year (free)

Select the option based on how many children you are signing up for Swim Team. If your child is returning, the cost is: 1 child = $100; 2 children = $200; 3 or more children = $250. If this is your child’s first year, they are free.
